• Detailed information about the event including the event date, location and number of people expected to attend
• Documentation of the organizations 501(c)(3) status

For a food donation request, please include the following:

• Name, contact information, and a brief description of the organization; including programs and services provided, and geographic area served
• The reason for the food donation request and purpose of the event
• Detailed information about the event including: The event date, time, address, number of people to be served, and amount of children that will be present
• Documentation of the organizations 501(c)(3) status
